Overview

This activity consists of two alternating parts, which both serve to address the gaps in individual and group practice: a best practice forum and a mortality review.

The forum brings together physicians and pharmacists for discussion of best clinical and educational practices related to inpatient and outpatient care with the goal of enhanced educational preparation for clinicians working with internal medicine (IM) residents and medical students. This portion of the activity focuses on 1) case-based presentations and discussion with the purpose of reviewing patients with diagnostic or management complexities, 2) review of evidence-based recommendations for clinical best practices and consensus, 3) educational strategies for development of clinician educators in the Billings Clinic IM residency program , and 4) sharing group medical practice considerations to improve practice performance and high-value care. Recommendations for content come from literature review, guidelines, expert clinician opinions, and practice data (M&M data).

The morbidity and mortality review includes IM physicians and a pharmacist and focuses on de-identified inpatient mortality case review to align inpatient medicine clinical practice with process improvement (PI) and best medical practices. The format includes a primary mortality review using the “Toolkit for Using the AHRQ Quality Indicators, How to Improve Hospital Quality and Safety,” case-based presentations to the IM inpatient faculty, and, if recommended, a secondary mortality review. If a secondary case review is warranted, an assignment is made within the IM inpatient provider group to further review and then bring back at a subsequent mortality review conference to recommend next steps, gaining insight from literature review, guidelines and/or expert peer opinions. The secondary review process may require an interdisciplinary approach. The patient population reviewed includes each inpatient mortality that occurs on any IM residency inpatient teaching service.


Objectives
At the conclusion of this activity, learners will be able to:

  1. Review patient cases that present diagnostic, management or educational dilemmas including a forum to enhance professional peer review in clinical circumstances surrounding mortality.
  2. Examine evidence-based literature and guidelines and recommend best clinical or educational practice changes in the care of patients.
  3. Disseminate best educational approaches to prepare providers to become proficient in clinical teaching strategies and in assessing new learners in ACGME core competencies.
  4. Explore group medical practice strategies to improve individual and group quality performance.
  5. Identify the need for process improvement using the AHRQ toolkit.

Billings Clinic is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.
